My work is blank, not saved

I downloaded a PDF worksheet from my college website. I filled out the form and saved it. I then uploaded the form on the college website. When my professor opened the form, it was blank. I went back to the form on my computer, it is also blank. I have 11 assignments that are all showing up blank. Does anyone know what could have happened or if it's possible to get my assignments back?

    What application did you use to open the file?

    It's possible the form is corrupt or there's a script in it that resets it. Hard to say for sure without seeing the actual file...
