Need help printing a PDF with a QR code

Greetings all,

Have a major headache, I have this fillable application with a QR code in it. It previews in Outlook 365 and on my iPhone perfectly, but if I open it in Adobe Reader or Print it, it converts the QR code to |||||0|||||0|||||||||0||||||

This is my second day working on this, Adobe Support was of no avail, does anyone have any ideas???
