pdf document with scroll tab isn't working

Hello, 

I'm running windows 10 and trying to figure out why a fill in PDF document form from my employer isn't working correctly. I can open it and fill in everything but one of two scroll down arrow boxes. Can someone open the document and see if it does the same thing on their computer.?
