PDF form 2D Barcode cannot generate on Mac, but generate on Windows

Somehow, when using Adobe Reader DC on Mac, the "PRINT FORM" button on the form ends up with "The document could not be printed." and "There were no pages selected to print." error messages when printing is attempted.  This works fine on Windows.  Please advice.
