pollution of Data in another location of the form

Hello,

I have enter the require information on a PDF form and the same information gets polluted into another section of the form which is an error and is not the same question how do I prevent this from happening.

Also I am unable to remove a check off of a check box so it is empty. How can I do this?

I am using Adobe Acrobat Reader DC

    Generally when that happens, it is because both of the form fields have the same name. This happens a lot when people that build the form copy/paste form fields but forget to rename them. Every field with the same name populates with the same data. This is intentional.

    There is nothing you can do about that using the free Reader. You should contact whomever gave you the form.
