Printing issues: some items don't print

I created a document in Microsoft Word that has text and "checkboxes." I exported the document to PDF because I ony have Acrobat Reader and not the full Acrobat software. I can "fill in" the document in Reader using the fill and sign tool.

When I print the document, the check boxes don't print. There is just a blank space. I can see the boxes onscreen in both the system viewer and the print preview. 

I have disabled enhanced security and printed it as an image. If I select print as an image in Reader, the boxes print but the black prints at only about 75% and the document is too light. When in print as an image mode, I choose any black and white printing options.

If I print using Preview, everything prints fine, but it is a hassle for me to have to open the documents in a separate application to print them.

I'm using a MacBook Air 2020.

Any thoughts?
