Problem with content in form

I have problem with document. The content of the form in document does not display correctly, but only on my computer (Win7, Acrobat Reader DC).

Why is it not displayed correctly? Like this:

It's likely the font used for the right text portion "ŁAPY-DĘBOWINA" hasn't been embedded into the PDF and doesn't exist on your computer, so Reader has trouble is displaying it correctly.

Fonts used in document are installed on PC.
