Radio Buttons not Displaying Chosen Answer in Fillable Form

When first formatting this fillable form a year ago, the radio buttons were working correctly and we didn't seem to experience any issue. Now, the radio buttons allow you to click on the options, but they do not record or display your choice. The box remains clear rather than filled. All of the check boxes and text fields seem to be working without any problems. I changed the setting to have the item fill color to be black. It seemed to work on one or two questions, but the rest, no luck. I'd like some guidance, please. As you can see, there are a lot of radio button options on this form, so ideally, I'd like to avoid having to redo the PDF and instead just edit the properties. Any guidance is appreciated. Thank you!
