Does anyone know of an extension or an Acrobat feature that allows one to change brightness, contrast within a PDF? Similar to Levels and Curves in Photoshop?

Well, not for the free Adobe Acrobat Reader (the focus of this forum), no. Sounds like you are working with a scanned image saved as a pdf? If so, you may try Photoshop or Photoshop Elements.
