Saving a pdf with an editable image field

Using Acrobat Pro DC, I have "added an image field" to my pdf as well as several text fields for the end user to personalize with their own photo and text information. I save my document as a Reader Extended PDF. Then using Acrobat Reader DC to test, I am able to replace the blank image field with a jpg photo and populate the text fields as I have designed. When I go to "Save As" my new pdf with the filled in properties, like the end user would do, I get an empty white image box while the text is properly displayed.

If this is not what an editable image field is for, why is it there? I get the same empty white box using Acrobat Pro to populate the image field, btw. What am I doing wrong?

    Your description is not very clear. Is the issue with the Save As dialog, or the image field? If the latter, does it only happen after you save the file? Please clarify.
