Text moves to other fields please help!

I have a pdf document that is fillable. I start to input my information and some of the text that I enter shows up in another field and then when I delete it out of that field it deletes in the other field I already inputted the info in. I have no idea what is going on and how to fix it. But this form has to be completed today!

    What's going on is that the author of the form screwed up. They copied and pasted the fields without renaming them or testing the file.

    As a result those fields have the same value. There's nothing you can do about it with the free Reader. Report this issue to the author and request a new version.
