What am I doing wrong??

I'm trying to fill up an application form (PDF) in Acrobat Reader DC. When I have to list something it automatically fills the different fields not allowing me to list them properly. I don't know if that makes sense???

    Yes, when I fill one field, this is then replicated in the rest of the fields below...

    You are doing nothing wrong. Fields with the same will hold the same content.
