Whenever I upload a photo to my document, it uploads to the other photo slots

I have to fill out a document that requires multiple images. However, when I select and image for 1 slot, it autopopulates that image for every other image. How do I prevent this from happening so I can upload each image in their correct slot?

Ask the author of the form to fix it. You can't do it yourself with the free Reader.

This happens when all fields have the same name. With Acrobat Reader you can't change this.
