Can you elabortate? Are you asking how to build a JavaScript application that uses PDF Embed but picks up the PDF to use via a query string value?

I mean that this link consists of three parts, the first part is the base
url, the second part is pdf_url, and the third part is pdf_name. For
example, at the present time, if I have a pdf file on my phone, what should
I do to configure such a link for it, and what are the steps that I need to
take to do that?

Your question really isn't making sense. There is no 'magical URL' that you would make that will do what you want. Instead, you have to build your own application that looks for the PDF and integrates it with Embed. 

In regards to your example of a PDF on your form, you can absolutely build a web app that lets a user click a button, select a file, and then pass that to Embed.
