URGENT Non digital Disappeared/Missing Signature - $4.2M business at Risk (and my job)

I am really hoping for a miracle. 

I am a single mom with 3 young kids and my job is at risk due to a technicality. 

I am a senior RFP manager at a medical device company. 

I submitted a bid response back in December 2023.

On the document, it specifies where I need to add my signature. 

Once the document is finalized, I save and recheck to ensure my (non digital or certifide) signature is there. As an extra check I add my initials to the bottom right hand corner to confirm that I signed the document. 

Well I found out on Tueday (my birthday) that we were disqualified due to a missing electronic signature on page 9.  My head is on the chopping block and I will possibly lose my job  tomorrow as they have scheduled a meeting at noon 😞  Which you know what that means. 

I am hoping there is an PDF Guru out there that can tell me what happened to my signed document. 

I just want to know if I honestly missed this signature ...or an error with the document.

Side note: I complete these files frequently and historically I have had issues with signatures disappearing (but caught before submission) or the signature would appear randomly on another page. 

I have attached the document.
