Data missing in pre-filled PDF form

Hello, I am integrating Adobe eSign using C# application. I have a pre-filled PDF form with name, city and other details. In which I have to add the signature fields at the bottom of the page. How to achieve it.

I created the transient document and agreement with the pre-filled form. But data(name, city, etc) is empty when the recipients got email for signature. Please suggest on how to keep the prefilled data after creating agreement.
