Do I understand correctly that you're creating an application form and what your applicants to upload attachments to that form? What are you using to create the form? If you're using Acrobat, please see https://acrobatusers.com/tutorials/how-to-create-a-button-form-field-to-insert-a-pdf-file. Or, if you Reader-enable the document in Acrobat, the user will be able to attach a file as a comment.

If you're using Adobe FormsCentral, you can add a File Attachment form field to your form. See FormsCentral Help | Create a form
