Batch transferring flat pdf to fillable pdf

The original PDF's I'm working with are flat. They'd be this form version https://edocs.dhs.state.mn.us/lfserver/Public/DHS-6532-ENG-dform but as a flat, nonfillable pdf. I need to move the field data to this form that is fillable https://edocs.dhs.state.mn.us/lfserver/Public/DHS-5788A-ENG-dform . The signatures would not need to be transferred. I'd like to do this to minimize the amount of manual input I'd need to do to get it right.

What are the best ways for me to go about doing this? I'm comfortable with running javascripts if need be. Thank you for your time in advance!!
