bookmark to document

The documents have been scanned in and bookmarks have been created. This is in Windows.  The bookmarks need to be pulled out and set up as a document.

Example files are scanned in, bookmarked as fruits, vegetables, nuts,berries,carbohydrates.  Each bookmark needs to become its own document which will then be uploaded to the client's file. How can this be accomplished.
