Check boxes with totals.... HELP!

Question

I'm trying to create something like this, with yes/no check boxes. Each yes/no answer has a value. The yes'no list then needs to go into a total box. I have a little experience setting up interactive PDFs, but the check box summations are throwing me off. Any help?

I'm assuming you have the full Acrobat, not just the online subscription version... You posted in the forum for Acrobat Online but it can't do what you're describing.

The simplest way to do it is to apply those values as the "Export Value" of each check-box and then use the built-in Sum option under the Calculate tab of your total script and select all fields.
