Creating a fillable PDF and saving it as a template

Hello!  I use a particular form that I create a fillable PDF for.  I have to change and delete many of the fields and want to save that as a template so I don't have to make those changes everytime I make this form fillable.  Can that be done?  Thanks in advance!
