Dynamic Stamp

Hi all, 

I am trying to create a digital stamp to automate an existing invoicing process where we use a phyical stamp and handwrite in certain required fields. I am using the stamps tools in Acrobat Pro to create a dynamic stamp where I can stamp the documents as I received them and enter in the required fields. 

This is the information I require on the stamp. I have created a stamp and then gone into in the prepare form tool to add in the text boxes so I can enter in the information. For whatever reason this hasn't worked out for me because each time I select the stamp it doesnt allow me to enter in the prepared fields. 

Any help on this will be greatly appreciated!
