Exporting form to pdf and tracking responses

I have Adobe Acrobat XI Standard and created a form using a template. When I try to pdf the form, the image of our logo isn't showing up and it's adding an extra page. How do I fix this? Also, if I distribute this form online, is there any way to link the responses to the email address that gets the notification?
