Form fill multi page document

i have created a 2 page form fill document that a record of shift notes are made & check box's, each double page represents 1 day and is named day 1- i have used the same template to creat day 2 - then combined the 2 documnets with the idea of havinging one file for several days worth of reporting..........the porblem i have is when you do a check box in day 1 it also fills in day 2, how can i stop this auto filling in same fields on different days or am i stuck having to have singular files for each day........also is there a way to prevent changes to the document once filled and saved, i've tried password protect but won't apply...i'm using Adobe DC.
