Form Filling

I am looking at developing a brochure so my client can fill out the questions on the brochure after they get the form emailed.  I want them to be able to email the form back to me and have it filled out.  What program do I use? How do I do it?

    If you're looking for a PDF Form, Please have a look at Adobe Acrobat.

    It will allow you to create forms, send them using acrobat.com account and receive response and manage them.

    You can email the form as well and collect response.
