Forms attached to emails cannot be filled out

Thank you for reaching out. 

We did check the PDF form, and we can fill it out without any issues on Chrome. However, there seems to be some issue with the submit button. Please try the steps suggested in the following help document to create a submit button: https://helpx.adobe.com/acrobat/using/setting-action-buttons-pdf-forms.html.
