How can I change a pdf to Fill & Sign form?

I have a form that can be printed and completed by hand. It is in a pdf format. I would like to put it into a Fill & Sign type of form. Does anyone know how to do this?

I have the same question.  And I have read the tutorial.  The problem is when I open a pdf document in Adobe Acrobat Reader DC, I can see the document, then when I click on fill and sign, the document disappears when the fill and sign screen comes up.
