How can I convert without losing document integrity?

I am trying to convert a pdf to word. The document has a lot of boxes(tables) and check boxes.I am new to this and am in a hurry for my agency to create a fillable document for report taking before Florence hits as our systems will be down but not our computers themselves. Any help would be greatly appreciated.

If you're referring to form fields they will be converted to static contents when you convert the file to another format.
