How do I turn a PDF into an auto fill document?

I am trying to turn a PDF into an auto fill doc, and having trouble. I have done it once before, but can't seem to start the process again???

I checked your account—you have both an Acrobat subscription and an Adobe FormsCentral subscription.

If you already have a PDF that you want to add form fields to, use Acrobat. Please see: Acrobat Help | Creating and distributing PDF forms
