I'm trying to fill and sign a PDF document

but when I go to save  and print it doubles up all the data input to the boxes. What am I doing wrong or is it the way the client has set up the PDF?

What software are you using to fill out the PDF form?  Is this with Acrobat/Reader, or the Fill & Sign tab in https://cloud.acrobat.com or something else?

I was using the Fill & Sign tab in https://cloud.acrobat.com
