Is there a template for creating structured questionnaires?

I'm trying to create a questionnaire where selecting a particular answer to a question would automatically 'jump' the user to the next relevant question, so they don't have to wade through lots of irrelevant questions. I'd also like to add guidance notes to each question. Is there a template that will allow me to do this? If not, how can I create such an interactive PDF?

    What product do you have for creating forms? There are some templates in FormsCentral (which is retiring in July). And Acrobat XI includes the FormsCentral Desktop app, so you can access the templates from there as well (choose File > Create  > Create Form, and then choose From Template to browse the available templates. If you don't find one that you like, you can create one from scratch. Here's some information about setting up conditional form fields, which is what I think you are asking about in your first sentence above: FormsCentral Help | Edit field properties
