    Hi;

    Can you give a little more detail on the workflow you are looking for?  Adobe Fill & Sign would not be the right solution but Adobe FormsCentral might do what you are describing but I need a little more info.

    In FormsCentral you can create web forms and in the web form you can have an "Attachment" field where the form filler can attach an image (or other file) to submit with the form.  You get all of the form submissions in your account in your "Response Table", form fillers do not log in and the forms are "publicly available".  FormsCentral also supports creating and distributing the form as a PDF to collect responses, but the PDF version does not support attachments.

    FormsCentral has two paid level subscriptions and also a Free level which allows you to create and distribute one form.

    Quickly & Easily Create Online Forms and Surveys | Adobe FormsCentral

    You can try it out free by clicking on "Sign In", selecting FormsCentral and then under the sign in Username and Password is a link to create a Free account.

    Thanks,

    Josh
