PDF merge to CSV Spreadsheet errors

I've had this incorrect spreadsheet data issue with both conversion from Word to fillable PDF and recreating a new pdf form from scratch (took a huge amount of time) in case there were issues with the original Word doc.   When merging to csv spreadsheet, some of the checkboxes in the report are not matching what was checked in the pdf form.  Some of the data fields are either showing "Yes" when a box was not checked or "Off" when it had been checked on the form.  Also, the last checkbox field on the spreadsheet is blank.  I checked all the properties for the form fields and removed all commas, dashes, etc. in case those would cause issues.  I've been working on this for two weeks now -- so frustrating.  Any suggestions would be much appreciated!

This is not possible with the Acrobat Online Services. What does you use?

I am using Adobe Acrobat DC.
