REQUIRED FIELDS ARE NOT POPULATING ON THE SUBMITTED DOCUMENT

We have web forms that we send out to customers to complete. The forms have required fields before they can sign to complete the document. My issue: When we get the completed form/packet, some of the required fields are NOT populating on the form. This issue does NOT happen on every form, which tells me the issue is NOT in the creation of the form.

EXAMPLE: Type of change: TRANSFER (condition set w/this selection to select current & new daycare)

Transfer from: Daycare selection           Transfer to: Daycare selection

Sometimes, the Transfer to choice populates when we get the form back, and sometimes it does NOT!

This is a new issue and just started occurring in the last several months. I figured it was a glitch but it has not corrected itself.

As you mentioned, you have a web format that you send to the customers. Please confirm if you are emailing the link to the web form to the customers or using the template to send for a signature. Also, did you check with customers to see if they have filled in the information?

Could you share the link to the web form or the template you have created? We will try to replicate the behavior for further investigation. Please also share the steps you take to send the document.

Please confirm if you are emailing the link to the web form to the customers or using the template to send for a signature. 

The link was created: 

1. We email the link to parents

2. Parents click the link via our website

 

Also, did you check with customers to see if they have filled in the information?

Answer: YES. The field is required. They cannot submit the form until all fields are completed. As I stated in my original post, we have also completed this form in office and it is still hit or miss.
