Uploading fillable form to LMS

I would like to have students fill in forms that have been uploaded to their LMS and to submit the completed form back to me.

How do I upload form into the course so that it remains fillable within the LMS?

What do I link the submit button to so that it returns to me as it would on a website?

Are there ways of putting in answers so that it becomes automatically graded?
