Want only one 'yes' box to mark at a time not all of the 'yes' boxes.

Working with 'yes' and 'no' check boxes in Adobe Acrobat Pro XI.  When marking the first 'yes' box, all other 'yes' boxes are checked.  How can I stop this and mark only one box at a time?
