What causes a conversion failure?

I scanned document (table + header)  to a pdf to convert to an xlsx file. However, I get an error message," Conversion Failure. The file has not been processed. An error has occurred which stopped the processing of the pdf"

Is there a way to scan & get the pdf processed?
