why when I create a form in adobe DC and I show all fields are but one field will not populate? I circled the field that will not populate

Try to change some spacing for this field in the original document and then convert it to PDF.

We did convert original doc to a word doc and then revised several times.

In the PDF, it showed the field as being there but would not populate. I

will see if I revise spacing a bit if that makes a difference.
