How to add embedded field into the certificate that I created using Express adobe?

I am trying to use the certificate and Qualtrics together.  Qualtrics has embedded data for the names but I don't know how to add the fields in the png file that I created in express adobe.  Also when I got the email that I was testing the image did not even show up in the email.  Also, how to make it print able in a email that I am sending?
