When opening Appended Files in Adobe Audition, the clips are not in order or some are missing

When I open Appended files in Audobe Audition the clips are sometimes not in order. 

Also when creating the unique copy of the audio, it sometimes misses clips despite all be selected.  

If anone knows why this is happening and how i can stop it I would be really greatful
