Accessibility: creating a custom TOC that includes a "skip links" type feature for screen readers

As we are aware, the Table of Contents generated by the skin editor reads incorrectly in a screen reader.  I welcome solutions to this problem.  I would like to create a a TOC that is visible on demand, with a "skip links" feature as one can create in HTML so the screen reader can efficiently skip to the main content.  Smart shapes don't work with screen readers, so please don't suggest that.
