Acknowledgement Forms in Captivate

Has anyone used adobe captivate to to create an acknowledgment form after a training course? We have an annual course where our employees need ot fill in a conflict of interest discloure form (agreeing they don't have a conflict of interest, or if they do they need to fill in what the conflict is).

 

Trying to see if there is a form template or if anyone has suggestions on how to do this.

    Does anyone have an anwer to this question. In my training I am having employees review a policy and I need them to be able to acknowledge it.   Is there a way it can be signed via Adobe signature and sent directly to HR?

    You would need a pdf in that situation if you really need a digital signature.

    This is exactly what Im needing as well. Also to be able to link the consent/discolsure form to survey results.
