How is it possible to add an image to a page with a multiple choice quiz question?

My apologies for the delay - we are trying to have pictures match with text - a "matching" sort of thing.

I would have suggested a Drag and Drop for this type of interaction. 

You could drag the text captions and drop them underneath their matching image. 

Or drag the images and drop them onto boxes above or below their correct captions.

@procuts2: Are you publishing to HTML5 or SWF?

If you need this drag and drop as a quiz question in SWF content you could perhaps consider using the Drag and Drop Question widget from Infosemantics:

Drag and Drop Question Widget (AS3) | Infosemantics Pty Ltd

It has the advantage over the default Captivate drag and drop slide in that it is a true quiz question and will also allow you to see the correct answer displayed in the Review Mode.  (Penalty Points however may not be an option.  And this widget is not as yet available for HTML5 output.)

If you need the image just as a decoratiing element for the page/slide itself, just drag&drop it.

If you need a specified image for each answer option (which I needed some time ago), I think you have to pass the shuffling of answers. In my former project I placed the images beside each answer option and deselected the "Shuffle Answer" option in the quiz preferences.

What is the purpose of the image? By default the embedded objects on a quiz slide have priority, they will always be on top of the stack. By embedded I mean those objects that have no separate timeline in the Master slides, nor in the slides.

the image is part of the quiz question - the idea is to match the image to the category

Therefore you´ll need a drag&drop interaction. You can include the d&d into your quiz.
