[Adobe Captivate 9] How to get data from mySQL to CP, and make comparison

Hi there,

I am trying to make a user info signup page in the CP9.

I implemented the database on a server via MySQL, and my PHP page looks like this. This shows how the form table is.

How should I store these variable cells into the MySQL, and also, later on, if I decide to ask the user to answer one of those questions, how should I pull the data from the MySQL from the external IP server and make the local comparison?
