Adobe Quiz Analyzer Not Pulling Organization

I am unable to pull my results into Adobe Quiz Results analyzer.  The two php files are in the root folder, but it will not pull the information into the program.  It looks to the other folders in the directory, but will not pull it from the CaptivateResults folder.  Notice the Organization gives me ".." and the department is the results folder:

It does not pull the correct CaptivateResults folder for the Organization / Department / Course.

If I place the folders from the server to my desktop, I can analyze all results, so the data is there.  Below is the results from my local computer but I would prefer to pull from the server so that I don't have the extra step:

Any suggestions?  I used the templates provided by Captivate 8, working off a Linux server and have both php files in the root folder:

Any help is appreciated.  I used the templates from Captivate and did not change them or add additional scripts.
