All items missing in quiz after save

I have many quizes saved in captivate. They are in a custom template and contain text, buttons, and images.

On saving and in some cases publishing the quizes (some but not all) will show blank if i re-open them to edit items. If i click where the text should be it will re-appear and i will have to go through each slide and click on every section that should show something for it to appear.
I have cleared my cache, i have done updates, i have re-created from scratch but noting seems to be working.
