Bad video quality

I have tried to download my video multiple times and when I upload it to the website,  My video needs to be in HD format and it is perfered to be mp4.  My video needs to be at least 720px. How do I set the perameters for this?

Also, my audio is messed up when I publish it to a mp4. It is off sync.
