Captivate 12.5 Simulation - Input fields give the failure pop-up on first click

Hello. Is anyone else having this issue? I have played around with it so many times and when I think its fixed I have someone else test it and it is happening again. The scenario is as follows: 

I have a input field for the end users to put in text and hit enter to move forward. I have disabled hint and correct answer pop-ups. When an end users clicks on the input field the failure pop-up triggers. It should only trigger if the end user inputs the wrong text. No click boxes on this screen either. Just the input field. 

At one point I was removing the input field, saving the simulation, clearing my cache, restarting, putting the input field back in (seems to save it as it was orginally added to the simulation even when i clear my cache) test it with successful results, saving it, testing again with successful results - giving it to someone else to test - and having the failure pop-up appear on the first click in the input field. 

Its not an end all but it makes the training seem wonky, and we all know we need our learners to trust the tech! Especially whe doing training. Help!
