Do you mean the Thumbnail dialog box which you open with the button Themes?  AFAIK the Themes menu itself never shows custom themes.

I always recommend clients never to store the custom themes in the default folder (Layouts in Public documents, eLearning Assets) but to keep them with the projects in the project folder.  You can always clean up that default folder by making a fresh copy from the original folder in the installation folder of Captivate. Don't forget that themes mostly have a companion Theme Colors palette as well.

The Themes menu WILL show custom themes if the custom theme files are saved to the correct folder in the correct folder where Captivate normally puts the default themes.

    E.g. C:\Users\Public\Documents\Adobe\eLearning Assets\Layouts\10_0\en_US

Check your system at that location to see if there are custom theme files there.
