Captivate 6 quiz issue - showing this phrase "You did not answer this question completely"

Hello,

I created a new layout and when I uploaded this to Taleo Learn site to test and publish, when I got to the quiz page, I was not able to answer any of the questions. Instead, I saw the phrase "You did not answer this question completely" at the bottom. I am hoping this is something in Captivate 6 I can easily fix.
